Healhtlink Massage Gift Card Terms and Conditions

  1. The use of the Healthlink Gift card is governed by these terms and conditions.
  2. If there is difference between the information/records on the Gift card and this form, the information/records on this form should prevail.
  3. This card is valid for 6 months from the date of issue. So the card holder should not delay using the card.
  4. This card is non-refundable and not redeemable for cash. If the card holder cannot use the card, breaks or loses it, Healthlink Massage is not liable for any loss or damage.
  5. This card is for booking only. When making a booking, the card holder should inform Healthlink Massage that they are the holder of this card.
  6. Healthlink Massage is not a medical institute and thus none of the information or services it provides can replace or be considered as medical advice or treatments.
  7. Customers are fully responsible to provide important information about their health and body, including advice from the customers’ GP or doctor about if the customer is suitable for a service Healthlink Massage provides, before having a service in Heathlink Massage. Healthlink Massage is not liable for any loss or damage in the case a customer fails to give this information accurately.
  8. By purchasing the gift card, I acknowledge that I have read and agreed with all the terms and conditions hereby stated and authorised Healthlink Massage to keep record of and use my personal information I have provided in this form according to its privacy policies.

Healhtlink Massage PrePaid Card Terms and Conditions

  1. The use of the Healthlink PrePaid card is governed by these terms and conditions.
  2. If there is difference between the information/records on the PrePaid card and this form, the information/records on this form should prevail.
  3. This card is valid for 12 months from the date of issue. So the card holder should not delay using the card. This card cannot be used in conjunction with any other long-term or short-term special offers Healthlink Massage may provide.
  4. This card is non-refundable and not redeemable for cash. If the card holder cannot use the card, breaks or loses it, Healthlink Massage is not liable for any loss or damage.
  5. This card is for booking only and cannot be used for services under the “TREATMENT” category. When making a booking, the card holder should inform Healthlink Massage that they are the holder of this card.
  6. Healthlink Massage is not a medical institute and thus none of the information or services it provides can replace or be considered as medical advice or treatments.
  7. Healhtlink Massage VIP Card Terms and Conditions

  1. These terms and conditions apply to all special offers stated on the VIP card and this form. The use of the VIP card is governed by these terms and conditions.
  2. If there is difference between the information/records on the VIP card and this form, the information/records on this form should prevail.
  3. Special options #1 can be used for card holders and their friends while the other options can only be used for the card holder.
  4. Special options #2 is valid only when the card holder has made a booking prior to coming in Healthlink Massage.
  5. Special options #3 stated in this card is valid only when the card holder has made a booking for Monday, Tuesday, Wednesday, or Thursday prior to coming in Healthlink Massage.
  6. Special option #3 is not valid in all Queensland public holidays.
  7. Bookings must be subject to availability. When making a booking, the card holder should inform Healthlink Massage that they are the holder of this card.
  8. All special options stated in the VIP card cannot be added up with each other or be used in conjunction with any other special offers/discounts/programs Healthlink Massage provides.
  9. Healthlink Massage Reserves its rights to modify and/or cancel all the special offers stated on this card without notice.
  10. Healthlink Massage is not a medical institute and thus none of the information or services it provides can replace or be considered as medical advice or treatments.
